Patient Instructions

Before Exam Day

1. Please make an appointment at the Colorado Preventive Medicine
(4700 E. Hale Parkway, Suite 120) approximately 3 weeks prior to your exam for blood testing.

2. Do not eat anything for 12 hours prior to your blood work. Please drink plenty of water to stay well-hydrated.

3. Allow 60 minutes to complete your ultrasound imaging and blood testing.

Exam Day

1. Please arrive promptly at the center at your scheduled time.

2. Free valet or free garage parking is available at the Hale Parkway entrance of the Rose Founder’s Building (4700 E. Hale Parkway).

3. No fasting is required on exam day unless you have been instructed otherwise. Please contact the office if you have any questions.

4. Please bring your medications including all vitamins and supplements with you.

5. Your detailed medical history, physical examination, EKG, audiometry, pulmonary function testing, and scans will take approximately 2 hours for established patients.  For first-time patients the entire exam and consultation will take approximately 3-4 hours.

6. At the conclusion of your exam, you will receive a complete file of your results, scan reports, and recommendations.

Payment

Full payment is expected at the time of your exam. We accept MasterCard, Visa, American Express, or personal check. Our primary care membership program and comprehensive health exam are not covered benefits of Medicare and Medicaid.  Depending on individual policies, private health insurance companies may or may not provide reimbursement.
